Filing a Claim
Our claim settlement procedure is prompt and hassle-free. It is advisable to consult our Customer Services team for detailed assistance while filing your J-STAR COVERAGE CARD Claim. Here is a simple step by step procedure to be followed when you make a claim.
STEP 1: WRITTEN INTIMATION
A claim can be lodged for benefits under the J-STAR COVERAGE CARD by submitting written intimation from the Card Owner/ Claimant (as the case may be) under his / her signature. Always remember to mention the event date, cause of the event and current address of the claimant while submitting written intimation.
STEP 2: COMPLETE THE CLAIM FORM ALONG WITH RELEVANT DOCUMENTS
Upon intimation of an occurrence (Within 07 days), J-STAR GROUP OF COMPANIES will furnish relevant forms for filing proof of loss which are required based on the nature of events. Different forms are to be filled for claims against riders, death, hospital cash benefit, group term insurance and gratuity.
Arrange for medical reports for medical related claims: In case of hospitalization or medical related claims, produce all medical bills and medical report issued by the attending physician qualified under law to issue such a report.
Attestation: Documents can be submitted in original or photocopies, attested by a Gazetted Government Official or a senior officer of our Company ( Branch Manager or above). Original copies of documents may be called for inspection.
STEP 3: SUBMIT REQUIRED DOCUMENTS ALONG WITH THE CLAIM FORM
The Original documents & forms (hard copy) properly completed, signed and witnessed must be returned to J-STAR GROUP OF COMPANIES within 15 days after the loss for which the claim is made.
STEP 4: SETTLEMENT
A claim is settled as soon as requirements to the satisfaction of J-STAR GROUP OF COMPANIES are completed.

In the event of a covered incidence occurring such as critical illness, hospitalization, disability or death, claim must be intimated to J-STAR GROUP OF COMPANIES within 07 days of such occurrence by writing to us at:
